New Team Application for 2011-2012 Season

Captains: Please note -- this form should not be used for teams that are playing in the current 2010-2011 season.

A "returning team form" will be available on the web site in May.  The form below is for NEW teams only.

As stated in Section V of the CALTA Charter, membership in CALTA is available to any team from the "Charleston Area", provided that the tennis center has adequate court availability, seating adjacent to the courts and restrooms available and open at 9AM on playdays.  For every two teams in CALTA, a tennis center must have available four courts for CALTA match play. For example, if a tennis center currently has two CALTA teams and a third team would like to apply, the tennis center must have eight courts available for CALTA play.  If a tennis center currently has four teams and a fifth would like to apply, the tennis center must have twelve courts available.

This form must be submitted by May 31, 2011.

Each team must have a minimum of eight players on its roster. There is no maximum.

In order to be included in the 2011-2012 schedule, a check for $240 (dues for 8 players @$30) must be received

by the CALTA Treasurer by May 31.  Dues are nonrefundable after that date.

While completing this form, do not hit ENTER to move to the next line.  Use the TAB key to move to the next box or click on each box.

If you hit "enter", the form will be submitted incomplete and you will have to start over.
